We know this is a difficult time. Here is some information that may make the arrangements easier for you and your family.
Our staff is available by appointment to help you arrange the final disposition of your loved one. Please call 727-1888 to set up a time to meet at your convenience. We encourage you to make an appointment so we can dedicate our time to you without interruption. We are available to assist you in planning everything from the simplest cremation to the most traditional funeral.
We will need information in order to process the Death Certificate and Permit for Burial or Cremation
Please bring the following information about your loved one to the arrangement conference with you:
- Full legal name
- Social Security Number
- Date and Place of Birth
- Parents names, including Mother's maiden name
- Usual occupation (we can not use retired)
- Spouse's Information
- Veteran's Information including DD214/Discharge Papers
- A solid color, high-necked shirt in a style that is most appropriate, if there will be a viewing.
If you would like an obituary please bring information on survivors. Children's names under the age of 18 will not be printed per PV Times. A photo can also be submitted. There may be a charge for obituaries in some papers.
If you would like to view your loved one please bring clothing to the arrangement conference and we will set up a time for you.
